What is Support at Home?

At Care Services Australia, we understand that staying in your own home is important for comfort, dignity, and wellbeing. Support at Home is a government-funded program designed to help older Australians to access the right care and support.

The Support at Home program replaces the existing Home Care Packages and Commonwealth Home Support Program, bringing everything together into one simple, easy-to-navigate system. It helps older Australians and their families access the right care and support, exactly when they need it.

How to Apply for Support at Home?

What is Support at Home?

To access a Support at Home package, you’ll first need to complete an assessment to determine your eligibility and care needs.

If you haven’t been assessed yet, you or your nominated representative can contact My Aged Care on 1800 200 422 to register and begin the process.

Understanding Support at Home

Under the Support at Home program, there are eight ongoing funding classification levels designed to match the level of support each older Australian requires. There is available funding classification from Level 1 (basic support) to Level 8 (highest support).

Transition from Home Care Packages

If you are currently receiving a Home Care Package (HCP), you will automatically transition to Support at Home. This means:

  • You will be approved for services under Support at Home without needing a new assessment.
  • Your HCP level will be mapped to the equivalent Support at Home classification.
  • Care Services Australia will continue providing your support, ensuring a smooth and uninterrupted transition.

With Support at Home, your funding is tailored to your individual care needs, giving you flexibility, security, and peace of mind while staying independent in your home.

Short Term Classification

In addition to your ongoing Support at Home funding, you may be eligible for short-term funding to help you stay safe, independent, and supported at home. These options are assessed and approved through My Aged Care.

  • Assistive Technology and Home Modifications Scheme – Helps older Australians live safely and independently at home. Access upfront funding for essential equipment, products, and home modifications that improve accessibility and wellbeing. You can receive funding for both assistive technology and home modifications at the same time if required.
  • Short-term Restorative Care Pathway – Focused on early intervention to help maintain or regain function and independence. Provides up to 16 weeks of intensive nursing and allied health services tailored to your needs. Each care episode can provide up to $6,000 in funding.
  • End of Life Pathway – For people with a life expectancy of three months or less who wish to remain at home with comfort and dignity. This pathway provides up to $25,000 over 12 weeks for in-home aged care services, in addition to any Palliative Care support already being received.

Will I have to contribute?

Under the Support at Home program, some services may require a contribution, while other services will be free of charge. Any contributions are calculated based on your income, assets, and the type and number of services you receive.

Financial hardship assistance

At Care Services Australia, we believe that financial circumstances should never be a barrier to receiving quality care and support. If you’re experiencing financial difficulty, you may be eligible for financial hardship assistance through the Australian Government’s Support at Home program.

How to Apply?

If you’re experiencing financial hardship, Care Services Australia can support you every step of the way. Our team can help you navigate the process and ensure your application is submitted correctly.

Here’s how we can assist:

  1. Confirm your eligibility – to see if you meet the government’s financial hardship criteria.
  2. Contact My Aged Care – We can help you get in touch with My Aged Care to discuss your circumstances and start the application process.
  3. Guide you through documentation – Assist you in completing the required forms and gathering the necessary financial details, such as income and asset information.
  4. Assessment and decision – Your application will be assessed by Services Australia, which will determine your eligibility and the level of financial support available.
  5. If your application is not approved – We can help you contact Services Australia to review or discuss the outcome and explore your options.

At Care Services Australia, we offer a range of services under the Support at Home Program to help older Australians live independently, safely, and comfortably in their own homes. Services are grouped into three key categories to ensure the right care for your needs:

  1. Clinical Support – specialised services designed to help you maintain or regain your physical, functional, and cognitive abilities. All services are delivered directly or supervised by trained, qualified or accredited health professionals. Our team is trained in evidence-based prevention, diagnosis, treatment, and management practices, ensuring safe, high-quality care tailored specifically for older Australians.

 

Clinical Support services include:

  • Nursing care
  • Allied health and other therapeutic services
  • Nutrition
  • Care Management

 

  1. Independence – Services provided to older people to assist with daily living activities and support the maintenance or recovery of skills needed for independent living.

 

Independence services include:

  • Social support and community engagement
  • Therapeutic services for independent living
  • Respite
  • Transport
  • Assistive technology and home modifications

 

  1. Everyday Living Support – Support to assist older Australians maintain a safe and comfortable home, enabling them to live independently.


Everyday Living services include:

  • Domestic assistance
  • Home maintenance and repairs
  • Meals
